DOUBLE BUNDLE ACL RECONSTRUCTION Most current ACL
reconstruction procedures focus on the reconstruction of the
Anteromedial bundle of the ACL with little regard for the Posteromedial bundle.
Recent studies have shown that this single bundle ACL reconstruction
sufficiently
restores the anteroposterior tibial translation but not the rotational stability
of the
knee joint and is associated with increased tibial torsion during running
activities.
These results prompted surgeons to devise a Double-Bundle ACL reconstruction
technique with the goal of achieving a more anatomic reconstruction of the ACL
and its two bundles improving knee stability and patient outcomes. Biomechanical
data seems to confirm that Double-Bundle ACL reconstruction provides better
anterior and rotational stability of the knee joint however long-term clinical
studies are yet to be published.
The procedure is done arthroscopically with hamstring tendons in a similar
manner to a single-bundle reconstruction and the rehabilitation and post-
operative course is also identical.
